Job Description:

Five ambulatory oncology Med/Onc clinics and infusion suites located in the Portland metro area. Chair/bed number ranges from 10-15 depending on site. Provide care to both Oncology and non-Oncology patients. Open 7 days a week including holidays and weekends.


The OHSU Clinical registered nurse (RN) provides compassionate, evidence-based, and efficient care to individuals, families, communities and patient populations. The Oncology Palliative Care Nurse Navigator will work collaboratively with referring providers, Palliative Care team and Knight Cancer teams to include Physician, surgeons, residents, fellows, RN coordinators and schedulers in the care of this complex population throughout the continuum of care and treatment. This position will work with the clinical nurse manager and other oncology nurse navigators to develop standard metrics, identify patient barriers, coordinate smooth transitions of care, and to be available for team members in determining and outlining the plan of care.  This position will collaborate with the other palliative care nurse navigators and nurse coordinators to cover days off and vacations.  An Oncology Palliative Care Nurse Navigator (NN) is a professional RN with oncology and palliative care-specific clinical knowledge who offers individual assistance to patients, families, and caregivers to help overcome healthcare system barriers.  Using the nursing process, a NN provides patient/family education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum.

 

The NN will work with the ONS and HPNA core competencies as the guide.  The NN competencies to be performed: (a) coordination of care, (b) communication to patient, families, and members of the healthcare team, (c) patient and family education  (d)  professional role.


  • External applicants: Baccalaureate of Science in Nursing Degree from a program accredited by Commission of Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN) or Commission for Nursing Education Accreditation (CNEA)
  • Internal applicants: Associate's Degree in Nursing
  • Current, unencumbered Oregon State Registered Nurse License
  • BLS at time of hire and must be issued by the American Heart Association (AHA) or Military Training Network branch of AHA (upload documentation with online application)
  • BLS must not expire during orientation or probation period, and must have the same expiration date (upload documentation with online application)
  • Complete ELNEC training within 12 months of start
  • Complete CAPC online training curriculum 12 months of start
  • OCN and CHPN certification within 24 months of start
  • Communication skills including fluency in oral and written English.Basic computer skills including the ability to send/receive email, navigate information technology associated with the position, and use Electronic Health Record information and tools
  • Minimum 2-3 years nursing in Palliative Care, acute care, oncology or ambulatory care
  • Knowledge of health and social service system
  • Excellent clinical judgment and critical thinking skills
  • Excellent communication and organization skills and attention to detail
  • Ability to work independently, as well as in groups
  • Must have excellent phone and customer service skills, the ability to multi-task and remain professional in a busy clinic setting
  • Excellent computer skills
  • Ability to document meticulously
  • Ability to communicate well with diverse populations
  • Must be able to perform the essential functions of the position with or without accommodation

Working Conditions:

 

The work environment carries risk of exposure to human tissue/fluids and radiation. Ability with or without reasonable accommodation corrective lenses and/or hearing aids to use the senses of seeing (including color vision ability), hearing, touch, and smell to make correct judgments regarding patient conditions and safely administer care. Use of all hospital approved clinical equipment. Significant use of computers, copiers, multi-line phones, and fax. Must be able to deal with competing needs and priorities of internal and external customers.

 


  • Current OHSU employees: BSN preferred
  • Knowledge of end-of-life care, including advance care planning, symptom management, and bereavement care is preferred
  • Experience with databases

Orientation: 4-8 weeks, commensurate with experience.
