Join our home health team and advance your home health nursing career in Oregon./n /n/nJob Details — Home Health RN / Travel RN/n /n - Position: Home Health Registered Nurse (RN) – Travel Assignment (Homecare Nurse)/n - Location: Salem, OR 97301 and surrounding areas (up to 75-mile travel radius including Canby, Molalla, Woodburn, Colton) — ideal for Salem home health jobs and Oregon nursing candidates/n - Start Date: March 30, 2026/n - End Date: June 29, 2026/n - Contract Length: 13 weeks/n - Schedule: Day Shift, Monday–Friday, 8:00 AM – 5:00 PM (5x8-hour days)/n - Hours per Week: 40/n - Estimated Weekly Pay: $2,050 – $2,160/n - Patient Volume: Average 5–8 patients/day; assigned 5–10 patients/day (productivity expectation: 30 points/week)/n - Travel: Up to 75 miles from home clinic; ~250 miles/week (mileage reimbursed at IRS standard rate for patient-to-patient travel)/n - Dress Code: Scrubs/n - EMR / Documentation: HomeCare HomeBase (HCHB) and PointCare; OASIS experience preferred/n - On-Call/Weekend Rotation: 1 weekend on-call every 4th week (varies by location; details discussed during interview)/n /n/nJob Requirements — Home Health RN Qualifications/n /n - Experience: Minimum 1 full year of recent Adult Home Health RN experience (or 1 year Hospice RN experience, depending on specialty)/n - License: Active Oregon RN license (must be in hand at time of submission; pending licenses not accepted)/n - Certifications: Current BLS and CPR required/n - Documentation Skills: Proficiency with OASIS and HomeCare HomeBase (HCHB) within the last 6 months, or willingness to complete retraining during compliance/n - Transportation: Reliable vehicle, valid driver’s license, and auto insurance (daily travel required)/n - Other: Must be open to on-call rotations and traveling up to 75 miles from assigned care center; first-time travelers not accepted; local candidates accepted at same rate/n - Productivity: Must meet weekly productivity points of 30 (equates to 40 hours/week); no guaranteed hours/n /n/nResponsibilities — Homecare Nurse / Home Health Nursing Duties/n /n - Provide, coordinate, and direct skilled home health nursing care according to physician orders and agency policies/n - Perform Start of Care (SOC), wound care, skilled follow-up visits, medication management, and other skilled nursing interventions in the home setting/n - Complete accurate, timely documentation in HCHB/PointCare; finalize all visit documentation within 24 hours/n - Assess, plan, implement, and evaluate individualized patient care and OASIS assessments/n - Educate patients and caregivers on disease management, medications, safety, and care plans to support home-based recovery/n - Supervise and evaluate Home Health Aides and LPNs/LVNs as applicable/n - Meet productivity expectations of 30 points per week (equivalent to 40 hours/week)/n - Participate in interdisciplinary collaboration and periodic office meetings; adhere to agency compliance, safety, and quality standards/n /n/n Why Salem, OR?
