Job Description & Requirements - Specialty: Physical Therapist - Discipline: Therapy - Start Date: 02/23/2026 - Duration: 13 weeks - 40 hours per week - Shift: 10 hours, days - Employment Type: Travel FACILITY INFORMATION Type of Facility: Critical Access Magnet / Teaching / Level Trauma (if applicable): Level IV Trauma Center Total Staffed Beds: 21 Scrub Color: Varies by Unit Charting: Epic Parking Cost: Free Physical Therapist - Outpatient/nYears of Experience REQ: 1 yr. 1 year of Epic experience is preferred./nCerts REQ: OR PT License and BLS required./nWeekend REQ: Mon - Fri preferred/nIs on-call REQ? No/nWill the clinician float within scope to meet facility needs? No/nFloating Req to Sister Facilities: No, but may be required to cover as an Admission Coordinator/Rehab PPS Coordinator/nOpen to accommodating block schedule? Yes, Mon-Thurs or Tue-Fri/nLocals Accepted: No/nGuaranteed Hours: 40 per week/nTop 10 Procedures/Scans: Ortho-general; total joints, acute/chronic pain; decreased balance; general neuro; general med; UE injuries; splinting necessary./nTypes of Insurance: All payer sources; primarily Medicare and Managed Medicare./nNumber of Cases /Treatments/Patients per day: 9 - 11 patients per day pending schedule/census/nWhat are the Productivity Standards: 0.45 and better for OP; 0.6 and better when split between IP/OP/n/n/n About LanceSoft Established in 2000, LanceSoft is a Certified MBE and Woman-Owned organization.
