Physical Therapist (PT) – Home Health- $5,000 Sign-On Bonus
Location: Eugene, OR (Field-Based Role) Work Type: Full-Time
Location Type: Hybrid (Patient Home Visits in Eugene and
Surrounding Areas) Base Pay: $58 – $68/hr- commensurate with

serving patients across multiple states is seeking a compassionate
and skilled Physical Therapist to join their team in the Eugene, OR
area. Known for delivering high-quality, patient-centered care,

of patients’ homes. As a Physical Therapist in this role, you’ll
develop individualized care plans, apply therapeutic techniques,
and collaborate with a multidisciplinary team to help patients
regain mobility and independence. This is an excellent opportunity

and assess patients’ physical conditions and functional
limitations. - Design and implement customized therapy treatment
plans to meet patient goals. - Administer therapeutic exercises and
manual techniques to restore movement, alleviate pain, and improve
strength and coordination. - Educate patients and caregivers on
proper movement techniques, safety, and home exercise programs. -
Collaborate with an interdisciplinary care team to ensure holistic,
high-quality patient care. - Maintain accurate, timely
documentation in accordance with clinical and regulatory standards.
Qualifications - Active Physical Therapist license in the state of
Oregon - Doctorate or Master’s degree in Physical Therapy - Minimum
of 1 year of Physical Therapy experience preferred (home health
experience a plus) - Current CPR certification - Reliable
transportation with valid driver’s license and current auto
