The Warmth of Family POSITION Community Nurse, RNDESCRIPTION
Position Summary: As the RN you will participate in the development
and implementation of health care policies and procedures. Promote
resident health and wellness in the community. Follow-up with
health care needs related to wellness, illness, or disability
within the RN scope of practice.RN Essential Duties and
Responsibilities:Input new resident data into the system and update
current resident's record.Participate in care conferences and
developing resident's Service Plan.Coordinate care with outside
providers and maintain appropriate documentation.Respond to
resident/family concerns identified and reported by Caregivers and
Med Techs.Document directions and actions appropriately, e.g.,
chart notes, nursing orders, and communication logPerform direct
nursing care as neededParticipates in providing continuing
education to staff, as assigned.Perform the act of Delegation and
teaching as determined by OSBN's Division 047.Member of CQI
(Continuing Quality Improvement) team.Investigate, evaluate, and
document incidents or accidents.Work collaboratively with other
care staff and management team.Must be self-motivated and able to
think independently.Perform assessments in a timely manner e.g.,
Pain, Diabetic, Supportive Device, Significant Change in
Conditions.Perform physical assessments of systems and report
abnormal finding to provider as needed.Update ADLs and communicate
changes with physicians and care staff, and families as
needed.Follow-up on lab reports and doctor orders, as per
policy.Serves as on-call RN for Community (on a rotating
basis).Respond appropriately to emergency situations.Rotating
on-call. Other duties as

Qualifications:Current Oregon RN licenseCurrent First Aid and CPR
certificationExcellent written and verbal communication
