The Clinic Social Worker provides social work services to patients who have been identified as needing ancillary mental health / psychosocial services. Works both independently and collaboratively as part of a multi-disciplinary team that includes Physicians, Pharmacists, Clinic Providers and staff, and other Clinic Care Management staff. Key services include (but are not limited to) psychosocial assessment; brief problem-solving counseling; crisis management; access to medications and insurance; referrals to community resources, education, and leading groups. Performs related work as assigned. Qualifications

Education/Experience: Master of Social Work degree from an accredited school of social work plus two years relevant experience (either pre or post-masters). Strongly prefer experience in one of the following areas:

• Care Management
• Mental Health
• Social Services Skill Requirement: Broad knowledge of the principles of social work and the ability to apply them, integrating knowledge of the normal aging process, changes related to chronic illness, and the needs of patients with chronic illness. Demonstrated problem solving skills and the ability to deal effectively with complex situations. Skilled at working with community organizations to access resources for patients. Strong advocacy skills. Excellent organization, oral/written communication skills for effective interaction with clients, clients' families, physicians and staff. Ability to organize and lead clinical groups. Able to foster communication and build relationships among Care Management staff members, providers and clinic staff.
